Form must be completed and submitted to TEAM MANAGER for submission to Club
Form must be on file with the club 24 hours prior to participation in any UOVASL event

TEAM MANAGERS & ASSISTANT FORM - Must complete an Admnistrator Registration even if you are a player
Pariticipation cannot be allowed without completion and submission of Registration Form
TEAM COACHES & ASSISTANT - Required for All Except Player Coach, all others required. Participation cannot be
allowed without completion and submission of Registration Form
ALL PLAYERS - Must complete Player Registration Form, submit to team manager for signature and forwarding to club
All player registrations must be accepted by team manager via his/her signature on form.
The online form can be completed online.
Complete the form, and save as yourname.doc then email to team manager for signature and forwarding to club
All new players must accompany the registration form with positive identification including birthdate and a
passport size photo for player book, or submit previous club player book.
Players previously registered must submit their old player book along with the registration form.
